This form is for submitting a specific video request to the Video Arts Team to be used in services, on the web, or in your ministry area.  Please provide as much information as you can, to help facilitate and drive our conversation with you about what exactly you need and when you need it completed.

Equipment and Maintenance

This form may be used to request any type of repair/maintenance for your technical systems by clicking on the button below and selecting “Tech Arts Repair/Maintenance” on the form. If you’d like to add a capability or piece of equipment that does not currently exist in your system, you may select “General Technical Arts Request” on the form.
